First-Pass Yield and Rework
First-pass yield makes hidden rework visible. A process can appear busy and productive while much of that activity is correcting earlier errors.

Define the pass point
Identify what 'correct first time' means at the point where output leaves a process step or end-to-end process.
Track rework loops
Record why work returns and which upstream condition caused it. Rework categories are useful inputs to root-cause analysis.
Quality and speed interact
Reducing cycle time by skipping necessary checks can lower first-pass yield and make total lead time worse.
